Medicare beneficiaries can now find Psynergy Health on Medicare.gov and enroll in coordinated chronic care at no cost, across all four ACCESS Model tracks. Patients can also sign up directly at psynergy.health/enroll or by calling 888-646-9587. CMS limited launch-day listings to participants already able to accept beneficiary sign-ups.
ORLANDO, Fla., Sept. 16, 2026 /PRNewswire/ — Psynergy Health, the AI-native clinical services arm of Andor Health, is listed in the CMS ACCESS Participant Directory that the Centers for Medicare & Medicaid Services launched today on Medicare.gov. The directory is CMS’s primary resource for Medicare beneficiaries who want to find an ACCESS Model participant and sign up for services. Starting today, a beneficiary, a family member, or a referring clinician can find Psynergy on Medicare.gov and complete enrollment online. Patients who prefer to enroll directly can do so at psynergy.health/enroll or by calling 888-646-9587.
Psynergy is enrolled across all four tracks: cardio-kidney-metabolic, extended cardio-kidney-metabolic, musculoskeletal, and behavioral health.
Today’s directory launch coincides with “Building a Healthier America: CMS ACCESS and a New Chapter in Health Innovation,” the CMS launch event being livestreamed at HHS.gov/live.
Three ways for a patient to start
A Medicare beneficiary can enroll through the CMS directory on Medicare.gov, directly at psynergy.health/enroll, or by calling 888-646-9587 and speaking with a care coordinator. Enrollment takes minutes and requires no paperwork from the patient. A coordinator makes contact within 24 hours. There is no cost to the patient for ACCESS services. A family member or caregiver can start the process on the patient’s behalf.
What the clinic does
Psynergy is a virtual clinic staffed by physicians, advanced practice providers, nurses, dietitians, and care coordinators who manage Medicare patients longitudinally between visits. Care is delivered where the patient lives, which matters most in the rural counties where the nearest specialist is an hour or more away. The ACCESS Model applies a payment adjustment for rural patients, and Psynergy built its program around that population from the start.
Patients enrolled in Psynergy’s chronic care programs have shown 92% blood pressure control within 8 weeks, 86% of patients reaching their clinical goal within 4 months, a 38% reduction in 30-day hospital readmissions, and over 90% active care plan engagement with an opt-out rate under 1%.
What it means for referring clinicians
A directory on Medicare.gov changes how a primary care physician or a hospital discharge team can act. A clinician can now point to a listed clinic on a government site and know the patient can enroll the same day, or hand the patient a phone number and a link. The referring clinician keeps the relationship. Psynergy returns a structured care update at initiation, at any escalation, and at the close of the care period, and any Medicare Part B clinician may bill the ACCESS co-management payment for reviewing that update and coordinating care. When a patient needs a higher level of care, Psynergy escalates within the patient’s own network so care stays local.
